I am a patient, how do I send my treatment form to Monaco’s health insurance funds?

Exceptionally, your treatment form will be sent on paper by the doctor who carried out the teleconsultation to the health insurance fund of which you are a member, using the unpaid fee procedure.

Your doctor will recover any patient contribution that is due.

You are not required to sign the treatment form.
